Traitement préventif :
2 000 UI/jour (risque moyen)
4 000 UI/jour (risque élevé)
Traitement curatif : 100 UI/Kg SC x 2 fois/24h IDM aigu avec sus-décalage ST, traité par fribrinolyse : 30 mg bolus IV puis 100 UI/kg SC x 2
Syndrome coronarien aigu non ST sus : 120 UI/kg SC x 2 fois/24h
Prévention de la thrombose chez l’hémodialysé (? 4 h) : 50 à 100 UI/kg dans la ligne artérielle du circuit de dialyse.
